Questions

How long is it?

Main story about 20-30 hours; 35-45 hours with side content; 45-55+ hours for completionists (reviewers range 40-50 h).

Can quests fail or content be missed, and what are the points of no return?

Yes. Launch reviews already flag missable weapons, optional regions and NPC steps (GamesRadar, Push Square, The Outerhaven). Side Tales can fail if you push the story. PowerPyx and Game8 both treat beating Bai Chilian, Lady of the Twin Serpents at the end of Crimson Forest as a hard lock: Game8 says Outpost and Thieves' Camp NPCs become Maskless and hostile, so remaining tale and NPC steps there are lost; PowerPyx names the Flesh Lily hand-in to Yu'er, A Healer's Mercy, Honor Among Thieves, Purge the Sect, Festering Secrets, and accepting Secrets of the Baishan Sect as failing after that fight. TheGamer, on Honor Among Thieves, warns of an unannounced point of no return where the Thieves' Camp changes, without naming Bai Chilian. Game8 is the only outlet we read that also names the Outpost in that conversion. PowerPyx puts an earlier cut on Apprentice Healer (fails once Jing Ye, The Outlaw falls); Game8 groups that Outpost tale with the Bai Chilian lock instead. PowerPyx also wants A Severed Mindseal's first step done before you leave Weeping Creek Valley, and The Journey Home and Young Master Lan's first steps before Pangzhen; we have not found a second publisher for those three cut-offs. A later gate: PowerPyx and Game Rant both name The Last Chance bell in Mount Soulrend. PowerPyx says it locks you into the finale until the final boss dies, then regions reopen for leftover non-missable work; Game Rant says after the credits you can load back to that bell (before the lock) or start New Game+. Game8's missables page lists only Bai Chilian as the "one crucial" point of no return and does not name The Last Chance. After the credits, PowerPyx says you can free-roam every area except the missable tales. Failed tales show as failed in the Journal; PowerPyx says reload a manual save or finish them in New Game+.

Is there New Game+?

Yes. A second playthrough unlocks Sixty-Six Days. PCGamesN says your equipment, progression items, ability training and archives carry over, and that quests, maps, key items and remaining days reset. In an RPGamer Q&A, director Soulframe Liang describes New Game+ as played "with all the equipment and level-ups".

Can I go back to Chapter 1 or earlier regions?

Sources disagree on when. AllThingsHow says Traverse only reaches Bells in the region you are in and cannot take you to another region. Game Rant says that once you reach the Village of Lu you can use any Bell to travel back to the Outpost in the first region, and TheGamer says visited Bells work even in completely different areas of the world. Either way the route is Traverse at a Bell to a Bell you have already touched; Game8 adds that a Traversal Talisman does the same from anywhere. PowerPyx says that after the final boss you can resume in free roam and return for what you missed, though some side tales fail as the story moves on: see the missables FAQ.
